Usually because the gutter is undersized for the roof feeding it, or because the pitch is wrong and water is not reaching the downspouts. Cleaning a gutter that is fundamentally too small will not stop it overflowing in a hard rain. That is a sizing problem, not a debris problem.
Usually repaired. Sag typically comes from failed or under-spaced hangers, or rotted fascia that will no longer hold a fastener. Fix the fascia and refasten properly and the run is generally fine.

Seams can be resealed, and that is often the right first step. But if you are resealing the same joints repeatedly, sectional gutters are simply doing what they do — at that point seamless replacement usually costs less over time than the ongoing repairs.

Yes. Rotted fascia is both a cause and a consequence of gutter failure, and there is no point refastening a gutter to wood that cannot hold a screw. We repair or replace it as part of the work.
Far less than what it prevents. Actual cost depends on the extent — resealing a few seams is minor; re-pitching a run or replacing rotted fascia is more involved. On-site assessment is free.
We will tell you straight. If the system is sized correctly and structurally sound, repair it. If it is undersized, chronically leaking at every seam, or hanging from failed wood, replacement is the cheaper answer over any reasonable time horizon.
